I work at run of the mill medical device company. I have a coworker adding dx codes with no documentation as to where the codes came from. In some cases said person does not even log their presence in the accounts but will show dx changes were made in the back end software by this person. I've found two instances orginally of the person updating codes to allow for processing and forwarded to management for compliance. They are insinuating that iam doing my own QA but aren't I liable if I see the dx changes and I do not report? I can across these first instances due to my own workflow in the first place. In normal practice, wouldn't said person be terminated on the spot? The excuse also is that we do not have a proper patient management software. We use something similar to Salesforce with another web based app. They stated due to some stipulation with the software, it's not fraud. I'm having a hard time getting this through my head and would appreciate any knowledge or advice on this. I've found about 10 instances so far on a quick search of modified dx codes by this . What would you do? I don't want to be fired or have to leave due to this mess. Thank you.

I'm seeing scary charts of the shortage of workers, and the increasing shortage at that, and thinking that there are hopefully some ways to try to ease the burden a bit. I think it's an industry that hasn't been shown enough love.
